HDL Cholesterol (mg/dL)

HDL cholesterol (HDL-C) measures the cholesterol carried by high-density lipoprotein particles, which transport cholesterol away from tissues back to the liver. People test it as part of a standard lipid panel because low HDL-C is associated with higher cardiovascular risk and often travels with insulin resistance.

The importance of HDL-C

HDL particles perform “reverse cholesterol transport,” collecting cholesterol from artery walls and tissues and returning it to the liver.[1] Observationally, higher HDL-C is associated with lower cardiovascular risk, though trials that raised HDL-C with drugs did not reduce events[2], so HDL-C is best read as a marker of metabolic context rather than a lever to push.

Low HDL-C rarely appears alone. It commonly clusters with high triglycerides, elevated fasting insulin, and abdominal weight gain, the signature of insulin resistance.[3] In that sense a low HDL-C is often a prompt to look at the whole metabolic picture rather than at HDL in isolation.

Regular aerobic exercise, weight loss, smoking cessation, and moderate alcohol intake are each associated with higher HDL-C, while smoking and metabolic syndrome push it down. If your HDL-C is under 40 mg/dL (men) or 50 mg/dL (women)[4], reviewing it with your doctor alongside triglycerides and glucose markers gives the number useful context.

What is a good HDL cholesterol level?

Commonly used reference ranges consider HDL-C of 60 mg/dL or higher favorable, 40–59 mg/dL acceptable, and below 40 mg/dL (men) or 50 mg/dL (women) low. Because cutoffs differ by sex and lab, compare your result to the range on your own report.

Is higher HDL always better?

Not necessarily. Higher HDL-C is associated with lower cardiovascular risk in population studies, but very high values (above roughly 90–100 mg/dL) have not shown additional benefit and in some studies associate with higher risk. HDL-C is most useful as one signal within a full lipid and metabolic panel.

How can I raise low HDL cholesterol?

Regular aerobic exercise, losing excess weight, stopping smoking, and replacing refined carbohydrates with unsaturated fats are each associated with higher HDL-C. Because low HDL often travels with insulin resistance, improvements in overall metabolic health frequently lift HDL-C as a side effect.

Why is low HDL often paired with high triglycerides?

Both patterns share a common driver: insulin resistance. When the liver exports more triglyceride-rich particles, an exchange process strips cholesterol from HDL particles, lowering HDL-C. Seeing the pair together is a common early metabolic signal worth discussing with your doctor.
